Getting Started with Stream

How do I set up Two-Factor Authentication? +

To set up Two-Factor Authentication (2FA) for your Stream account:

  1. Log into your Stream account
  2. You'll be prompted to set up 2FA
  3. Open your authenticator app and scan the QR code
  4. Enter the code from your authenticator app into the prompt
  5. You should be logged in

Remember to keep your 2FA device accessible to ensure you don't lose access to your account.

If you get a new phone, reach out to support@riverrecords.ai for help setting up 2FA again.

If you don't have a two factor authentication app, we recommend Google Authenticator for iPhone or Google Authenticator for Android.

How to use Stream:

What do I need to use Stream? +

To use Stream, you need:

  • A device with a web browser (computer, tablet, or smartphone)
  • Internet connection
  • A microphone (built-in or external)
  • Your Stream account credentials

Using Stream

How do I start a new encounter? +

To start a new encounter:

  1. Log into your Stream account
  2. Click on "Start New Encounter" in the top right corner
  3. Select the patient (or create a new patient record)
  4. Choose the encounter type and adjust additional details as needed
  5. Click "Continue"

How do I add a new patient? +

To add a new patient:

  1. Log into your Stream account
  2. Navigate to the "Patient List" tab
  3. Click on "Add Patient" or select "Add Patient" when starting an encounter
  4. Fill in the required patient information and click "Add Patient" or "Continue"

How do I generate documentation? +

To generate documentation:

  1. Complete your patient encounter
  2. Click "Generate Problem List"
  3. Review, edit, and confirm the problem list
  4. Apply templates if desired
  5. Click "Generate Cards"
  6. Review and edit the generated cards as needed

How do I record on mobile? +

To record on mobile:

  1. Login to your Stream account
  2. Click "Start New Encounter" or "Record"
  3. Start Recording
  4. Stop Recording when finished
  5. Save the recording
  6. Return to the patient list
  7. Generate documentation on a larger screen (tablet or computer)

How do I edit or correct information? +

To edit or correct information:

  1. Locate the card containing the information you want to change
  2. Click on the card to open it
  3. Make your changes by typing directly in the card
  4. Alternatively, click "Record" to add a voice note with corrections
  5. After adding a voice note, click "Refresh" on the card
  6. Review the updated information

How do I bring forward information from a previous encounter? +

To bring forward information from a previous encounter:

  1. On the problem list, locate the problem containing the information you want to bring forward
  2. Click on the card to open it
  3. Click "Copy" to copy the information

Customization

What are the differences between Classic Soap, Problem Based, and Fully Combined notes? +

Stream offers three different note organization styles:

  1. Classic SOAP:
    • Organizes information in the traditional Subjective, Objective, Assessment, and Plan format
    • All problems are discussed together within the Subjective and Objective sections
    • Each problem addressed gets its own Assessment and Plan sections
    • Best for simpler visits or when you prefer traditional documentation
  2. Problem Based:
    • Organizes information by problem, with each problem having its own complete discussion
    • Each problem includes relevant subjective, objective, assessment, and plan information
    • Ideal for complex visits with multiple problems. Great for review!
  3. Fully Combined:
    • Each section in the SOAP contains information from every problem
    • Best for EHRs that only allow one S, O, A, and P sections each
    • Efficient. These note styles tend to be the most brief

You can select your preferred note style when generating documentation. The style can be changed for each encounter based on your needs.

How do I bring forward information from a previous encounter?

How do I create a template? +

To create a template:

  1. Navigate to the Templates section
  2. Click "Add Template"
  3. Give your template a title
  4. Click the light bulb icon for AI-suggested structure (optional)
  5. Add custom instructions or structure as needed
  6. Save your template

How do I apply a template? +

To apply a template:

  1. After confirming your problem list for an encounter
  2. Click the template dropdown next to the problem you want to apply the template to
  3. Select your desired template
  4. The template will be applied when you generate the card

Account Management

How do I change my password? +

To change your password:

  1. Navigate to the Profile section
  2. Click "Change" under the password section
  3. Enter your current password
  4. Enter your new password and confirm it
  5. Click "Change Password"

How do I change my email? +

To change your email address, please email us at support@riverrecords.ai.

How do I cancel my subscription? +

To cancel your subscription:

  1. Navigate to our billing partner's website
  2. Enter your email address associated with your Stream account
  3. Wait for an email from Stripe (our billing partner)
  4. Click on the link in the email to manage or cancel your subscription
  5. Your account will remain active through the end of the billing period. You will maintain access to your records. We will turn off your ability to generate new recordings and documentation after the billing period has ended.

Troubleshooting

What if the AI makes a mistake? +

If you notice an AI mistake:

  1. Review all generated content before finalizing
  2. For small errors, edit the card directly
  3. For larger corrections, add a voice recording with natural langauge instructions that explain the correction
  4. Click "Refresh" on the card to update with new information
  5. Always double-check the corrected information
