Simply upload a PDF or paste text from any outside record, and Stream automatically extracts the relevant information — problems, medications, key findings — and files each item into the right problem buckets for you.
Discharge summaries, referral letters, and old consult notes become structured, problem-linked documentation that you can actually use. The next time you see the patient, you’re not digging through attachments — you’re continuing the story from where someone else left off.
This feature will also form the foundation of Stream’s Chart Builder, allowing you to reconstruct a patient’s longitudinal record even when they come from multiple external systems.
